This PR adds a few issues templates as well as a pull request template.

Seeing as there are only two issue templates included by default, is the expectation that we will preserve the option for users to use a blank issue?

It seems like there are a number of PR types that might not be covered by bug-report/feature-requests, so I assume that's the intention. If so, then I approve this.

Correct, I mainly wanted this to be easier for the most common types of issues, but preserving blank issues is definitely my plan.
